• Possibly name and telephone number of relatives
• Information about health and illness for use during treatment at Hair Transplantation Clinic TALIZI
• Receives information from own doctor, insurance companies, ministry of health, relatives
• Sends to own doctor, statutory databases, insurance companies, ministry of health.
• We follow the rules in the Journal-keeping and book-keeping Act.

If you apply for a job with us, the following personal data is processed:
• Name/address/age/family connection/education/previous employment.                                      
• This is stored while the employment procedure is on…
• We do not store unsolicited applications.
• When employed, the personal data is also stored. and possibly illnesses that may affect employment. In addition, name and contact information for next of kin. Authorization no. + information on salary,criminal record, tax + pension. This is stored throughout the employment
• In addition, minutes from interviews are stored incl. mouse throughout the employment.
• Work schedules/salary information are stored for five years.

Your rights
• You have the right to gain insight into which personal data we process about you
• You have the right to have the personal data we have registered about you corrected and updated
• You have the right to have the personal data we have registered about you deleted you. If you wish to have your personal data deleted, we will delete all information that we are not legally required to store.
• If the processing of personal data is based on your consent, you have the right to withdraw the consent, which means that processing will then cease, unless we are required by law to process the personal data.

Security
All employees at Hair Transplantation Clinic TALIZI have a duty of confidentiality and confidentiality is stipulated by law (Section 27 of The Law of Georgia on patient’s rights). The duty of confidentiality also applies after termination of employment.
